Insurance Process

I participate in many major insurance plans, although coverage for services varies by plan. Please provide full insurance information prior to your initial visit so that I can begin the insurance verification process. If you have a change in insurance, let me know as soon as possible to avoid surprise charges.

I encourage you to verify your benefits with your insurance company prior to your appointment.

Out of Network Billing

I do not bill out of network benefits, as it often results in a surprise bills to clients with inconsistent reimbursement. If you have out of network benefits that you would like to use, I can provide an itemized receipt that you can submit directly to your insurance to request out of network reimbursement.
